How we collect, use and protect personal information in line with the Protection of Personal Information Act 4 of 2013 (POPIA).
Last updated: 28 October 2026
1. Who we are
LifeBridge Health and Rehabilitation ("LifeBridge", "we", "us") is the responsible party for personal information collected through this website. Contact us at 46 Worraker Street, Newton Park, Gqeberha, 6045, on 041 8800 372 or at info@lbhrc.co.za.
Registered company name and registration number: [to be confirmed]
Information Officer: [name of Information Officer, to be confirmed], contactable at [email address, to be confirmed].
2. Scope
This policy covers personal information collected through this website, including the contact form, email and telephone enquiries that start from the website, and the website's visitor counter. Personal and health information processed during medico-legal assessments is handled under separate engagement terms and consent, which we provide to claimants and instructing attorneys directly. [Confirm that such terms exist and are provided.]
3. Information we collect
Information you give us
When you use our contact form we collect your name and surname, email address, contact number, the type of enquiry, your role (if you choose to share it) and your message. Please do not send confidential medical information or records through the website; we will advise on a secure method if documents need to be shared.
Information collected automatically
- Visitor counter. To count website visits we set a small first-party cookie (
lb_visit) that expires after 30 minutes of inactivity, so that browsing several pages counts as one visit. We do not store your IP address. To avoid double-counting, a one-way hash of your IP address and browser type, combined with a random value that changes every day, is kept for that day only and then discarded. This cannot be used to identify you. - Server logs. Like most websites, our hosting provider automatically records technical information such as IP address, browser type and pages requested, for security and troubleshooting. These logs are kept by the hosting provider for a limited period. [Confirm retention period with hosting provider.]
- Spam protection. To prevent abuse of the contact form, a one-way hash of the sender's IP address is kept for up to one hour to limit repeated submissions.
We do not use advertising cookies, tracking pixels or third-party analytics on this website.
4. Why we use your information
- To respond to your enquiry and, where you request it, to arrange assessments and appointments.
- To keep a record of our correspondence with you.
- To keep the website secure and to understand overall visitor numbers.
We process enquiry information on the basis of your consent and to take steps you have asked us to take. You may withdraw your consent at any time by contacting us, which will not affect processing already carried out.
5. Sharing your information
We do not sell or rent personal information. We share it only where necessary: with the medical experts involved in coordinating your matter (where you have asked us to arrange assessments), with our website hosting and email service providers who process it on our behalf, or where required by law.
6. Third-party services and cross-border transfers
Our contact page includes an embedded Google Map. When it loads, Google may receive technical information such as your IP address, under Google's own privacy policy. Our website fonts are hosted on our own server. Our hosting and email providers may store data on servers located [in South Africa / outside South Africa, to be confirmed]. Where information is transferred outside South Africa, we take reasonable steps to ensure it is adequately protected, as required by section 72 of POPIA.
7. How long we keep it
We keep enquiry correspondence for as long as needed to respond to you and for our legitimate record-keeping, after which it is deleted. [Confirm retention period, for example 3 years.] Visitor-counter hashes are discarded daily, and rate-limiting hashes within one hour.
8. How we protect it
We use reasonable technical and organisational measures to protect personal information, including encrypted connections (HTTPS), restricted access to stored data and secure email delivery. No method of transmission over the internet is completely secure, so please avoid sending sensitive information through the website.
9. Your rights
Under POPIA you have the right to:
- ask whether we hold personal information about you, and request access to it;
- ask us to correct or delete information that is inaccurate, irrelevant, excessive, out of date or obtained unlawfully;
- object to the processing of your personal information on reasonable grounds;
- withdraw consent you have given; and
- lodge a complaint with the Information Regulator.
To exercise these rights, contact us at info@lbhrc.co.za. We may need to verify your identity before responding.
10. Complaints
If you are unhappy with how we have handled your personal information, please contact us first so we can try to resolve it. You may also contact the Information Regulator (South Africa) at inforegulator.org.za, or by email at POPIAComplaints@inforegulator.org.za.
11. Changes to this policy
We may update this policy from time to time. The latest version will always be available on this page, with the date it was last updated.
